For registration in the Darien Public Schools, complete the attached registration and health forms and also bring with you to your child’s school the following: child’s birth certificate, immunization record and two forms of proof of residency (valid CT driver’s license and  one of the following: current utility bill, signed lease/rental agreement or current mortgage statement).
