Athletic Release Form - Coaches at Darien High School are often asked by College Recruiters to release academic and athletic information about their players. Since this is protected information, we are required to have a signed authorization on file before we can release information about your child.
If you would like to authorize someone in the Darien High School’s Athletic Department to release this type of information, please print and complete the linked PDF and return it to the Athletic Director's office.
New SPORTS PHYSICAL rules for High School Athletes from the CIAC and Darien Board of Education...
All interscholastic ATHLETES (High School only) must, in accordance with CIAC rules and the Darien Board of Education, have a yearly SPORTS PHYSICAL current within 13 months of the beginning of tryouts, practice and competitive play. This physical must be kept updated and on file with the school nurse. We cannot allow a student to tryout, practice or continue competitive play once this physical becomes more than 13 months old. The white sports physical form is available on the Darien Public Schools website, from the coaches, and in the high school health office.

ImPACT Concussion Management
In Fall 2010 Darien high School Athletic and Health Services Department implemented the ImPACT Concussion Management Program.
Each student athlete participating in interscholastic athletics at Darien High School is encouraged to take a baseline Impact screen prior to the first day of tryouts. In compliance with DHS head injury guidelines and Connecticut State law [PA No. 10-62 Substitute Senate Bill No. 456] if while your child is participating in interscholastic sports and is observed to exhibit signs, symptoms or behaviors consistent with a concussion following an observed or suspected blow to the head or body.
