Athletics Forms

 

1. DHS Coaches’ Athletic Emergency Form - All Athletes need to complete the form and print one copy, sign and return to the DHS Health Office.
2. State of Connecticut Department of Education Health Assessment Record – All athletes need to print ONE copy, complete the form, sign and return it to the Health Office. Athletes must return the form on the first day of tryouts.
