- Emergency Form - Complete the form and print ONE copy. Return the signed form to homeroom or the school nurse on the first day of school.
- DHS Coaches’ Athletic Emergency Form - Only Athletes need to complete the form, print one copy, sign and bring to the first day of tryouts.
- State of Connecticut Department of Education Health Assessment Record – All athletes and ALL 10th GRADERS need to print ONE copy, complete the form, sign and return it to the Health Office. Athletes must return the form on the first day of tryouts. 10th Graders must return the form to the health office during the school year (before entrance into 11th grade).
- Pay to Participate - The Darien Public Schools require that each student participating in a Darien High School sanctioned club, sport or activity pay a $100 participation fee per student, per academic year. Students choosing to participate in an extra-curricular activity or on an athletic team will also be asked to abide by the DHS Extra-Curricular And Athletic Eligibility And Participation Contract. Payment of the fee and electronic signature of the participation contract should be submitted through PayDeck. PayDeck opens for 2011-12 payments on August 15th.
- Electronic Environment User Agreement: All students will be required to read and sign the district’s user agreement by October 1st. Students will be prompted to sign the agreement electronically via the Aspen student portal on or after August 29th.
- Nondisclosure form - This is not a required form unless you want to opt out of allowing student work and images from appearing in school publications and websites.
Senior Parking
- Senior Parking - Seniors may apply for a parking permit the week BEFORE school starts. See the application for details.
Health Information and Accident Insurance
- DHS Health Office Introduction - The Darien High School health office is staffed by two full time nurses. There are school health practices and procedures that may impact your child. Most often these requirements are based on Board of Education policy as well as state and local legislation. Please review each of the items listed within to determine applicability to your child.
- Connecticut's HUSKY Plan - Free or low-cost health Insurance for children and teenagers.
- Student Accident Insurance Letter - Student Accident Insurance is available to all students in the Darien Public Schools.
