Physical Examination Requirements for Newly Enrolled Students into DPS:

Pre-kindergarten students entering preschool must have a health assessment documented on the yellow State of Connecticut Early Childhood Health Assessment Record. The health assessment must be dated no earlier than 365 days prior to school entrance. For this coming school year (2008-2009), the physical must be dated on or after August 29, 2007.

A student entering kindergarten, transferring from a private school in the U.S. (including CT) or transferring from any public or private school outside of the U.S., must have a health assessment documented on the blue State of Connecticut Department of Education Health Assessment Record (or its equivalent) prior to school entry per Connecticut State Law C.G. S. Sec 10-206. The health assessment must meet the mandated screening/test requirements of Connecticut State Law C.G. S. Sec 10-206. The health assessment must be dated no earlier than 365 days prior to school entrance. For this coming school year (2008-2009), the physical must be dated on or after August 29, 2007.

A student entering from any U.S. public school (including CT), must submit a health assessment documented on the blue State of Connecticut Department of Education Health Assessment Record (or its equivalent) prior to school entry. The student may submit a health assessment done 365 days prior to school entrance or a health assessment dated during the year the child entered kindergarten, sixth grade or tenth grade, if the health assessment meets the mandated screening/test requirements of Connecticut State Law C.G. S. Sec 10-206.

No student will be admitted in the Darien Public Schools until a school nurse has reviewed the new entrant’s completed health assessment and immunization records.


Physical Examination Requirements for Students Currently Enrolled in DPS
:

All returning Darien Public School students entering grades six (6) and ten (10) must have a health assessment as per Connecticut Law C.G. S. Sec 10-206 current within the same calendar year and documented on the State of Connecticut Department of Education Health Assessment Record (or its equivalent). The health assessment must meet the mandated screening/test requirements of Connecticut State Law C.G. S. Sec 10-206.  For the school year 2008-2009, the physical must be dated on or after January 1, 2008.  All sixth and tenth graders must submit this physical examination form to their school nurse during the school year and before entrance into seventh and eleventh grade.

Mandated Screening/Test under Connecticut State Law C.G.S. Sec 10-206

  • A health and development history as physician feels is necessary and appropriate.
  • A chronic disease assessment: which shall include, but not limited to asthma
  • Hematocrit or hemoglobin test
  • Height, weight and blood pressure
  • Vision, hearing, postural, gross dental screenings
  • It is recommended that an assessment be made of the risk of exposure to tuberculosis. If the child is at high risk, a Tuberculosis Skin Test is required with the health examination. In addition, students originally from high risk countries that are entering CT schools for the first time, must receive a Tuberculosis Skin Test.
  • Documentation of complete up to date immunization at the time of entry into the Darien Public School System in accordance with state law C G S 10-204a and the State of CT Department of Public Health Immunization Requirements for Students in Connecticut public schools. (CT Department of Public Health Requirements included in packet)

Exceptions to the school entrance immunization requirements:
      1.   Certificate from your physician stating the medical reason an immunization is contraindicated for the child.  This is forwarded to, and must be approved by, the CT State Health Department. Please allow plenty of lead time.

      2.   A statement from the parent or guardian that such immunizations are contrary to the child's religious beliefs.


Please note:

Physical examinations performed outside of the United States are not accepted. Physical examinations performed outside of the state of Connecticut must be signed by a medical doctor. All immunizations must document month, day and year.

Sport Participation Physical Health Assessment (Sport Participation Physical) for all interscholastic ATHLETES (High School only):

Student athletes participating in an interscholastic sport must, in accordance with CIAC rules and the Darien Board of Education, have an annual Sports Participation Physical current within 13 months of the beginning of tryouts, practice and competitive play. This Sports Participation Physical must be kept updated and on file with the school nurse. At no time shall an athlete be allowed to practice or compete if the sport participation physical date exceeds 13 months. Even if the sport participation health physical was accepted at the beginning of the season, if during the season it becomes more than 13 months old, the athlete will not be allowed to continue to practice or play until a new sport participation physical is on file with the school nurse. The sport participation physical should be documented on the DPS Sport Participation Health Record.

Please note the requirements for the sport participation physical examination and the Tenth grade State of Connecticut Department of Education Health Assessment are different. One is not a substitute for the other.
