Health Requirements for Registration

We welcome you and your child to the Darien Public School District (DPS). Below please find important information on health documents required for your child to be enrolled.
Health Assessment Record: All students enrolling in DPS must have a physical examination before they may enter school per Connecticut Law (P.A. 94-103, Section 10-206). The physical examination must be documented on the State of CT Department of Education Health Assessment Record form (HAR-3) (also known as the “blue form”) or its equivalent, and must include the following criteria:
- Mandated screening / test under Connecticut State Law C.G.S Sec 10-206 (denoted by an asterisk (*) on the HAR form
- Up-to-date immunizations record per State of Connecticut Guidelines State of CT Immunization Requirements.
- Performed and signed / stamped by a legally qualified health provider (including a medical doctor, advanced practice registered nurse, or physician’s assistant)
- Completed Part 1 of HAR (health history) and signed by parent / guardian
Note: Any physical exam performed outside the United States is accepted ONLY if performed by a healthcare provider licensed to practice medicine in the United States.
Students who are transferring from a public school in the US (including CT) must submit a health assessment record from a physical exam performed within 365 days prior to entry into DPS or within 365 days prior to when the student entered Kindergarten, 7th or 9th grade.
Students who are transferring from a private school in the US (including CT) or transferring from any public or private school outside of the US must submit a health assessment record with an up to date immunization record from an physical exam performed within 365 days prior to entry in DPS.
Students who are entering Kindergarten must submit a health assessment record with an up to date immunization record from a physical exam performed within 365 days prior to entry in DPS. Students who are entering Pre-Kindergarten must submit a health assessment record upon entry into the Early Learning program (ELP) and ANNUALLY THEREAFTER. Physical examinations must have been performed no earlier than 365 days prior to entry into DPS. The physical examination must be documented on State of CT Department of Education Early Childhood Health Assessment Record (also known as the“yellow form”) or its equivalent, and must meet the above criteria.
