Health Requirements For Enrollment into Darien Public Schools

Physical exams with up-to-date state mandated immunizations are required at the following times:

1. ELP: Exam within the calendar year prior to students entry into Pre-K (submitted annually). For information specific to Early Learning Program registration click link- click here for incoming ELP Letter 2024-2025

2. Kindergarten: Exam on or after August 27th 2024 prior to students entry into Kindergarten. For information specific to Kindergarten registration click link- click here for incoming Kindergarten letter 2025-2026

3. 7th Grade: Exam on or after June 30th 2024 prior to students entry into 7th grade

4. 10th Grade: Exam on or after June 30th 2024 prior to students entry into 10th grade.

6. Students receiving itinerant services: Exam within the calendar year prior to students receiving itinerant services (submitted annually). For information specific to itinerant services registration click link - click here for incoming students receiving itinerant services letter 2024-2025 

The physical examination must be documented on the State of Connecticut Health Assessment Record for K-12 students, also known as the "blue form" or its equivalent, and must include an up-to-date immunizations record* per State of Connecticut Guidelines State of CT Immunization Requirements.  Please be sure to have Part 1 (health history) completed and signed by the parent/guardian.

For Pre K / Early Learning Program (ELP) students, the physical examination must be documented on the Early Childhood Assessment Record, also know as the "yellow form" or its equivalent and must include an up-to-date immunization record per State of Connecticut Guidelines State of CT Immunization Requirements.   Please be sure to have Part 1 (health history) completed and signed by the parent/guardian.

Email all required forms in a pdf format or mail hard copies to you child’s school nurse prior to the start of the school year.

Students who participate in DHS interscholastic athletics must submit a valid physical exam per CIAC guidelines when registering under FamilyID for participation in high school interscholastic athletics.
