Health Requirements

Health Requirements For Enrollment in Darien Public Schools

Physical exams with up-to-date, state-mandated immunizations are required at the following times:

  1. Early Learning Program (ELP): Exam within the calendar year prior to the student’s entry into Pre-K (submitted annually). For information specific to ELP registration, click here: Incoming ELP Letter 2025-2026.
  2. Kindergarten: Exam on or after August 26, 20252, prior to the student’s entry into Kindergarten. For information specific to Kindergarten registration, click here: Incoming ELP Letter 2026-2027.
  3. 7th Grade: Exam on or after June 30, 2024, prior to the student’s entry into 7th grade.
  4. 10th Grade: Exam on or after June 30, 2024, prior to the student’s entry into 10th grade.

The physical examination must be documented on the State of Connecticut Health Assessment Record for K–12 students, also known as the “blue form” or its equivalent. The form must include an up-to-date immunization record in accordance with the State of Connecticut Immunization Requirements. Please be sure to have Part 1 (health history) completed and signed by the parent/guardian.

For Pre K / Early Learning Program (ELP) students, the physical examination must be documented on the Early Childhood Assessment Record, also know as the "yellow form" or its equivalent, and must include an up-to-date immunization record per State of Connecticut  Immunization Requirements.   Please be sure to have Part 1 (health history) completed and signed by the parent/guardian.

Email all required forms in a pdf format or mail hard copies to you child’s school nurse prior to the start of the school year.

Students who participate in DHS interscholastic athletics must submit a valid physical exam per CIAC guidelines when registering under FamilyID for participation in high school interscholastic athletics.
